Can someone please check this and rewrite it in a good way?

I couldnt sleep because I was having a heartache so I went to the doc to get checked. He checked me and told me that I have this love disease. He wrote me a perscription and handed to me. I looked and saw your name on it. The doctor told me to hurry and go find it. I need to take it ASAP or I would die.
